During your visit, we will:

• Review your dental and medical history forms
• Diagnose your dental pain and discomfort
• Discuss X-rays and other scans
• Create your own individual treatment plan
• Answer any questions you may have

 

Bring your smile, but also remember to bring these with you to your appointment:

• Your completed patient information form (if you haven't completed it online)
• Your insurance card
